Disección aórtica tipo B e insuficiencia renal aguda en un paciente con enfermedad renal crónica

Resumen

Se presentó un paciente de 39 años de edad, raza blanca, con antecedentes de hipertensión arterial sin tratamiento regular y manifestaciones de isquemia en miembros inferiores con claudicación intermitente. Se diagnosticó disección aórtica tipo B. Los síntomas al ingreso fueron dolor precordial con irradiación abdominal, que se interpretó como una cardiopatía hipertensiva y cólico nefrítico. Luego, presentó deterioro rápido de la función renal con necesidad de tratamiento hemodialítico de urgencia e hipertensión arterial de difícil control. Falleció a los 30 días a causa de trombosis mesentérica con necrosis intestinal extensa.

ABSTRACT

A 39- year- old, male, white patient was presented in this paper. The patient presented a hypertension without regular treatment and manifestations of lower limb ischemia with intermittent claudication. A type B aortic dissection was diagnosed. Chest pain with irradiation to the abdomen was the symptoms found at the admission. And a chronic coronary artery disease and renal colic were involved in the diagnosis at the beginning, 24 hours later an acute kidney injury with necessity of hemodialysis treatment as emergency treatment, and hypertension of a very difficult management were diagnosed. He died 30 days after he has been admitted by a mesenteric thrombosis with an extensive necrosis of the intestine.
